It wasn't just ITV4 that went down, every ITV channel (except CITV, who were already closed for the evening) had issues. There's a lot more info in this thread from the blue place:

I find it odd that somehow they managed to get on the 'programmes start at 6pm' slide (When at this point ITV4 had gone 24/7), two CITV slides (The CITV Channel would originally timeshare with ITV4 from when CITV launched in March 2006 to when ITV4 went 24/7 in February 2008. Unless the timeshare is anything like the timeshares between CBBC/BBC Three and CBeebies/BBC Four where both channels shared presentation areas until ITV4 went 24/7, then I don't know how that happened), an ITV Play slide (Why would they still have that on the system? By this point it had been a year since ITV Play closed) and the Men & Motors slide from when it closed on Freeview.
