My understanding is that Sky have a policy of delisting channels which stop broadcasting within a few days, but will relist them if they come back on air within a certain longer period. There were periods when Information TV and Showcase TV (and maybe some others) disappeared while they had a carriage issue (meaning bills hadn't been paid) but came back after they'd been resolved.
As for what counts as broadcasting and not, barkers/loops definitely don't otherwise the 'ghost BBC Three' stuff that went out at 2am wouldn't have been necessary.
Not sure if the specifics of the policy are stated publicly but this is my understanding.
