I suspect like most of these things Sky just didn't want to pay any more than they absolutely had to for the format in 2008, in fact I wouldn't be surprised if they bought it straight from America and repackaged it themselves, rather than jumping through ITV loopholes...
That being said, there was a soundtrack of some sorts from Gladiators released, so they'd done a Millionaire long before Millionaire, in releasing the bulk of the original music, so much like when F1 coverage moved around the terrestrial networks, The Chain followed with it (Sky didn't use The Chain, and have never used it AFAIK), being a commercial piece of music (and covered as it probably was at the time by the BBC's music blanket).
Music can be expensive to clear and it can be cheaper just to ask somebody to create something similar to the original but just different enough to be unique. Or maybe the BBC have bought the original tracks. The graphics will be new, doubtless, but, well time will tell.
