Now, the current version is a lot closer to the proper logo, but if you look at the file history and look at the earliest uploads, you can see the culprit - the typeface is too thin and the C is too high up.
You see I don't think that necessarily is the culprit; I'm not sure of the original upload date - but these titles were used from May 2006 with the offending logo on them:

I do wonder if the person who made the wikipedia version copied it from a programme, or worse - the original uploaded was a BBC employee who made it and thought it was good enough.

Funny thing is that you could probably balls up your recreation of the Reith logo and still have it look fine.

Hmm that doesn't look quite right but it looks more like it's been squashed a bit, rather than having the tell-tale thin typeface and C too high up.
There probably wasn't a vector graphic version of the BBC logo online at the time, and Wikipedia has always prioritised vector graphic versions of logos rather than JPEGs, PNGs etc, so I honestly think that's where it originated. Made with good intentions, but then used on actual productions.

Episodes 2 and 3 had the correct logo though (episode 4 isn't on till tomorrow).
Episode 4 had the 1997 logo on too... did they possibly make versions with both logos, possibly not being entirely sure when the episodes would air or the new logo would come in, and UKTV just got copies of the first version they picked out? Those two episodes definitely had the 1988 logo on original broadcast as I remember watching them.

Look North Hull currently has the old BBC logo on screen as Peter Levy talks to someone about a lack of NHS dentists.

It's not great is it... I do wonder how these things happen, unless they've used an ancient PaintBox and the new logo was too big for the floppy disk...

I must say that is a rather interesting way to present the DTL (correct me if I'm wrong :p) though, rather than the standard plain background
At school they taught me how to be
So pure in thought and word and deed
They didn't quite succeed...

Or the usual of having the subject displayed on the set?

It's one of those things that Look North kept even though they now have the new set. They sometimes use the wall, but for some topics they'll use the split screen, there is a generic version, but it is "usual" for them to make a background tailored to the story. When presented in this way, they also have an alternative name strap that appears within the confines of the guests side of the split.
